About the Role:

As a Care Coordinator, you’ll support individuals with complex needs by assessing, planning, and coordinating services—primarily for those with opioid use disorder. You’ll connect clients to resources, monitor progress, and advocate for their continued care and recovery.

What You’ll Do:

  • Conduct comprehensive client assessments
  • Develop and manage individualized service plans
  • Coordinate services and community resources
  • Monitor client progress through regular contact
  • Maintain accurate documentation and records

Who We Are Looking For:

  • Bachelor’s degree in healthcare, psychology, social work, or related field
  • 1+ year of care management experience (SMI, SUD, or similar populations)
  • Valid Florida driver’s license with clean record
